Job DescriptionSalary: $23.00 per hour

EA Family Services is hiring a full-time Lead Care Manager for the Sacramento area. This position would entail working with a dynamic team providing care coordination/case management for Medi-Cal enrollees. Must love working with people; have a respectful and objective attitude; be friendly and outgoing. Must have a BA/BS in the social services field. Looking for Bilingual speaking both English/Spanish and Just Involved Lived experience, referring to a person having personal experience with the Justice system, including an individuals personal encounters, interactions, and involvement with the legal and judicial aspects of society. Must have a history of working with the Justice-volved population. The hourly pay is $23.00 DOE. Great benefits. Must pass DOJ/FBI/CAIC clearances.


Qualifications:

1. BA or BS degree in social work, psychology, or similar social science

2. At least two years experience in a social services setting

3. FBI, DOJ, Child Abuse Index Clearance (CAIC), Health Screening and TB clearance

4. Current Drivers License and clean driving record

Duties and Responsibilities:

1. Provide consultation, case management services, and guidance to ECM clients, and liaison services with other agencies per each clients Individualized Needs and Services Plan (Case Plan).

2. Co-develop an Individualized Needs and Services Plan with each assigned client, other team members, and the clients identified family/social supports.

3. Assist in formulating SMART goals, implementing the case plan, supervise its progress, and make revisions as needed.

4. Initiate and maintain communication with other agencies and members of the treatment team for each client.

5. Make weekly contact and monthly face-to-face visits (or as often as necessary) as part of Individualized Needs and Services Plan, transport participants when necessary, and monitor cases of approximately 25 clients (but up to 50 possible).

6. Write timely progress and quarterly reports, as well as required case notes.

7. Administer the ANSA Assessment upon admit and every six months thereafter.

8. Review and monitor housing units to ensure compliance with all applicable laws and regulations.

9. Develop a Transition Plan for each client, which includes post-planning and instruction.

10. Inform Administration of any illegal or harmful (or potentially so) situations regarding the operation of programs and clients.

11. Follow State and Federal laws regarding confidential information and child abuse reporting.

12. The position is designed to accommodate the changing schedules of clients and may require on-call responsibilities.

13. Other duties and responsibilities as required by the Executive Director(s), Program Director(s), or a Supervisor.
