Responsibilities

  • Deliver direct patient care within scope of LVN licensure on-site.

  • Assess patient conditions, document findings, and report changes promptly.

  • Administer medications and treatments per prescribed protocols.

  • Coordinate with nursing and medical teams to ensure seamless care delivery.

  • Maintain accurate electronic health records with timely updates.

  • Monitor clinical quality metrics and adhere to regulatory standards.

  • Meet or exceed performance targets through effective prioritization and patient throughput.

  • Identify opportunities for process improvements and contribute ideas.

  • Engage in ongoing professional development to advance clinical skills.

  • Communicate care plans and outcomes to patients, families, and team members.

  • Maintain a safe, organized, and compliant workspace on-site.

  • Support pipeline-based planning by documenting patient flow and capacity.


Experience/Qualifications

  • Active LVN license in the applicable state with current practice status.

  • Provide direct patient care within LVN scope on-site, following protocols.

  • Accurate documentation of assessments, interventions, and patient outcomes in EHRs.

  • Administer medications and treatments per prescribed orders and safety checks.

  • Coordinate with nursing and medical teams to ensure timely care delivery.

  • Maintain regulatory compliance and monitor quality metrics related to patient care.

  • Demonstrate strong patient throughput and prioritization to meet performance targets.

  • Identify process improvement opportunities and contribute ideas for workflow enhancements.

  • Engage in ongoing professional development to advance clinical skills and licensure.

  • Communicate care plans and outcomes clearly to patients, families, and team members.

  • Maintain a safe, organized, and compliant on-site workspace and equipment.

  • Document patient flow and capacity to support pipeline-based planning.

  • Availability for contract employment with on-site location and typical shift coverage as defined.

  • Ability to work in a growth-oriented, metrics-driven environment with merit-based rewards.
