ACI has produced positive savings in every performance year since the program began in 2021. Our purpose is to improve patients’ quality of life, including spending more days in the place they call home, and help the practices caring for them thrive.

This role is a frontline point of contact to key practice partnerships, ensuring that those practices are supported, and that ACI represents its work with one voice. You will be the day-to-day account leader for your book of business. This will include coordinating regular meetings, managing messaging, going to the practice to build relationships and understand their operations, ensuring scorecards and reports go out on time, without error and, most importantly, managing follow-through and making sure nothing slips.

By enabling our practices to succeed in VBC, you will drive their financial success through improving shared savings performance. You will be supporting our practices in delivering excellent care to their patients. Reporting to the Senior Director,Practice Success, this is a key role in a newly developed Practice Success team, with room to grow responsibility and impact over time.

Salary range will be $120,000-$138,000 total compensation, including annual bonus based on company and individual performance. Compensation for the role will depend on several factors, including a candidate’s qualifications, skills, competenciesand experience.

Core responsibilities

- \n
- Own ~15-25 accounts as the key POC and internal voice of the practice.
- Onboard new practices and run QBRs
- Produce practice scorecards and own the follow-up tracker so every committed action closes.
- Review key insights and surface to the customer to generate tangible, measurable performance improvement – coordinating internal SMEs on solutions and execution guidance to the practice.
- Run practice-facing roster intake for provider additions and departures on the CMS calendar, delivering clean, complete files to ACO Operations for submission.
- Own and manage SFTP folders for data exchange.
- Execute practice-level outreach campaigns end to end, such as EMR integrations, SDOH data collection, voluntary alignment campaigns, etc.
- Coordinate pilot rollouts at the practice level and maintain the measurement tracking behind them.
- Maintain the reporting cadence and data hygiene (CRM updates).
- Provide day-to-day coordination support on practice relationships owned by the Senior Director: responsiveness, scheduling, notes, and follow-through.
- Coordinate and manage in-person meetings with key accounts.

- Your assigned partners generating positive savings
- Uptake by practices of recommendations from ACI, with tangible/measurable performance improvement (measured as a percentage of committed actions closed)
- Partner retention – targeting 100% for all controllable elements,
- Positive NPS reviews, positive informal feedback from partners, and strong referral pathways to net new partnerships

Who we are looking for
\n
- \n
- 4 to 7 years in healthcare customer success with Value Based Care experience.
- Proof of retaining customers and growing existing partnerships
- Data and reporting fluency: understand the analytics we share with the practice
- Polished, professional communication with practice managers and physicians.
- Learns technical program mechanics fast and stays organized across many simultaneous deadlines.
- Experience in operations and/or project management is a plus.
- Experience working within a practice or health system in Operations or Customer Success is a plus
