RN Case Manager
Location: Milford, MA
Rate: Up to $2,300 Gross Weekly + benefits available from day 1
Shift: Days, 5x8-hour shifts, 8:00 AM–4:30 PM; every third weekend required
Contract: 10 weeks
Hours: 40 hours/week
Requirements:
- 3–5 years of experience required
- BSN required
- Massachusetts RN license required
- Acute care experience required
- Case management/utilization review experience
- Care coordination and discharge planning experience
- Epic experience preferred
- Local and travel candidates accepted
Certifications:
- BLS required
- CCM preferred
Responsibilities:
- Provide case management services for adult and geriatric patients
- Coordinate patient care and discharge planning
- Complete patient needs assessments and coordinate DME orders
- Support care coordination across the acute care setting
- Apply CMS and HIPAA guidelines to patient care and documentation
- Collaborate with interdisciplinary teams to support safe and appropriate patient outcomes
