Role Description The Physical Therapist role at Ivy Health is a full-time, on-site position based in White Lake, MI. This role involves evaluating patients, developing and implementing individualized treatment plans, and delivering evidence-based physical therapy interventions to support functional recovery and improved quality of life. Day-to-day responsibilities include performing comprehensive assessments, documenting clinical findings, monitoring patient progress, and adjusting treatment plans as needed. The Physical Therapist collaborates with referring providers and interdisciplinary team members, educates patients and families on home exercise programs and injury prevention, and upholds clinic standards for safety, ethics, and patient satisfaction. Participation in team meetings, ongoing professional development, and quality improvement initiatives is also expected.

Qualifications

- Demonstrated expertise in Physical Therapy and Rehabilitation to assess, plan, and deliver effective treatment programs.
- Strong Patient Care skills, including compassionate communication, clinical judgment, and individualized treatment planning.
- Experience or familiarity with Medicine concepts relevant to musculoskeletal, neurological, and orthopedic conditions.
- Ability to support Home Care or home exercise programs through clear instruction and ongoing patient education.
- Graduate degree in Physical Therapy (DPT or equivalent) from an accredited program.
- Current state licensure (or eligibility for licensure) as a Physical Therapist in Michigan.
- Solid documentation skills and proficiency with electronic medical records.
- Ability to work collaboratively in a multidisciplinary team and maintain professional, inclusive interactions with all patients and colleagues.
- CPR certification and adherence to all applicable regulatory and ethical standards.
