Doctor of Pharmacy (PharmD) degree, active Pharmacist license, and minimum 1+ years Pharmacist experience required. Applicants who do not meet these qualifications will not be considered.

This role focuses on the critical responsibilities of a home infusion clinical pharmacist, who prepares, dispenses, and monitors complex intravenous (IV) and subcutaneous medications for patients in a home setting. As a key member of a multidisciplinary healthcare team, this professional ensures safe and effective medication therapy, provides patient and caregiver education, and maintains regulatory compliance.

Responsibilities:

  • Prepare specialty pharmaceuticals and IV solutions in a sterile, ventilated environment adhering to all state and federal regulations.
  • Assess lab values and calculate pharmacokinetic dosing (e.g., vancomycin), monitor Total Parenteral Nutrition (TPN), and adjust treatments accordingly.
  • Provide comprehensive patient counseling on specialty medications and treatment plans, ensuring understanding and adherence.
  • Educate patients and caregivers on infusion pump usage, sterile techniques, and catheter maintenance in the home setting.
  • Collaborate closely with prescribing physicians, home health nurses, and internal teams—including intake, referral services, reimbursement, nursing, clinical services, and pharmacy operations—to facilitate seamless therapy management and prevent hospital readmissions.
  • Maintain strict compliance with State Board of Pharmacy regulations, HIPAA, FDA, DEA, Medicare, ACHC, URAC, OSHA, and other relevant standards.
  • Manage pharmacy inventory, including refrigerated products, and prepare medications and supplies for secure, timely home delivery.

Qualifications and Desired Experience:

  • Current, active NY pharmacist license in good standing.
  • Minimum of 2 years’ experience in infusion specialty pharmacy.
  • Experience with IVIg preferred.
  • Strong knowledge of specialty therapy payer requirements, reimbursement processes, and contract obligations.
  • Familiarity with the CareTend system is a plus.
