Bilingual (Spanish-Speaking) LVN – Care Coordination / Non-Bedside

Position Overview We are seeking a bilingual (Spanish-speaking) Licensed Vocational Nurse (LVN) for a non-bedside care coordination role. The primary objective of this position is to monitor high-risk patients — including our highest-acuity patients and frequent ER users — to help prevent unnecessary hospitalizations and emergency room visits. This role also provides support for quality-related initiatives and assists the front and back office during urgent situations or emergencies.

Key Responsibilities

Quality & Care Coordination

\n

    \n
  • Make daily, weekly, and weekend outreach calls to the clinic's highest-risk patients (including Band 5 patients and frequent ER users) to triage concerns and assess social, medical, and emotional needs.
  • \n

  • Submit referrals to insurance-approved resources and route specific patient needs to the appropriate provider.
  • \n

  • Schedule patients for urgent appointments as needed.
  • \n

  • Conduct outreach calls to patients who may be at risk of needing emergency care.
  • \n

  • Follow up on provider-directed calls for patients flagged for additional monitoring.
  • \n

  • Document and submit ER and hospital admission/discharge information through the appropriate portals.
  • \n

  • Attend periodic meetings with the Office Manager to present patient cases to providers for evaluation and guidance.
  • \n

  • Contribute to and help implement strategies for reducing hospitalizations and ER visits; proactively identify and suggest process improvements.
  • \n

  • Conduct weekly outreach calls to Band 5 (highest-acuity) patients.
  • \n

  • Support Transitional Care Management (TCM) by making weekly follow-up calls for four weeks post-discharge and scheduling appointments within 7 days of discharge.
  • \n

  • Assist as needed with quarterly preventive screening campaigns (e.g., Cologuard, breast cancer screening, diabetic eye exams), including reviewing patient lists, contacting patients, and sending orders to appropriate centers or labs.

  • Serve as a flexible resource for the front and back office, providing coverage as needed.
  • \n

Qualifications

\n

    \n
  • Active LVN license
  • \n

  • Bilingual — fluent in Spanish and English
  • \n

  • Strong communication and organizational skills
  • \n

  • Comfortable with phone-based patient outreach and care coordination
  • \n

  • Experience with EMR/portal documentation preferred
  • \n
