Home Infusion Clinical Pharmacist Compensation
- The pay for this position is $64.27 per hour
- Benefits are available to full-time employees after 90 days of employment
- A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates
Home Infusion Clinical Pharmacist Highlights
- This position is a contract assignment with potential to hire on permanently based upon attendance, performance, and business needs
- The required availability for this position is Monday – Friday with rotating shifts:
8:30am – 5:00pm, 9:30am – 6:00pm, 10:30am – 7:00pm - Must have flexibility to work all schedules and on-call evenings/weekends as needed
Home Infusion Clinical Pharmacist Responsibilities
- Evaluate and monitor drug therapy for home care patients in collaboration with physicians, nurses, and healthcare professional
- Adhere to all organization policies, procedures, and applicable state/federal pharmacy regulation
- Ensure compliance with accreditation standards and participate in quality assurance activiti
- Coordinate new referrals and admissions with nursing, customer service, and distribution team
- Review, receive, and verify new patient medication orders and document appropriatel
- Assist physicians and healthcare teams with patient care planning and therapeutic drug therapy goal
- Conduct initial and ongoing patient assessments to identify drug-related problem
- Monitor drug therapy outcomes using lab data, clinical data, and patient information
- Provide consultation to patients, families, and healthcare professionals regarding medication use
- Maintain accurate and complete clinical documentation in patient records
- Process, check, and verify medications, supplies, and equipment prior to dispensin
- Compound sterile and non-sterile preparations including parenteral nutrition, antibiotics, chemotherapy, hydration, and analgesic
- Ensure compliance with USP 797 and USP 800 cleanroom and compounding standard
- Document all clinical and operational activities in required logs and system
- Maintain clinical knowledge through ongoing education, literature review, and trainin
- Supervise pharmacy technician functions as required by Pharmacy Board regulation
- Support workflow operations, inventory control, and medication distribution
Home Infusion Clinical Pharmacist Requirements
- Bachelor of Science in Pharmacy required
- Active Registered Pharmacist license in state of practic
- Minimum of 2 years recent acute-care experience OR 1 year home infusion experienc
- MUST HAVE HOME INFUSION EXPERIENCE
- Experience with sterile compounding and infusion therapi
- Strong organizational, time-management, and communication skill
- Ability to work flexible schedules including evenings, weekends, and on-call as neede
- Must be able to sit, stand, walk, and perform keyboarding tasks; occasional lifting of 10–30 lbs
- High School Diploma or GED
- Attendance is mandatory for the first 90 days
Home Infusion Clinical Pharmacist Preferred Qualifications
- Experience with CPR+ or other pharmacy management system
- Prior home infusion pharmacy experience strongly preferre
- Membership in professional pharmacy organization
- Strong clinical assessment and pharmacotherapy knowledge
