Key Responsibilities:

Escort residents to and from clinic appointments.

Arrive at work a half hour before the scheduled clinic visit to assist residents in preparing for their appointments.

Ensure proper nursing uniform and ID badge are worn at all times, both at the facility and during clinic visits.

Assist residents with pre-appointment needs and ensure they receive their snacks for the outing.

Monitor residents to prevent falls and ensure a safe, peaceful environment, intervening immediately if necessary.

Report any incidents or accidents to the Clinical Coordinator or ADNS promptly.

At the clinic, obtain relevant reports, paperwork, and supplies for the resident’s visit, ensuring the necessary paperwork, such as DNR/DNI forms, accompanies the resident.

Act as a proactive advocate for the resident during clinic visits, addressing any issues by promptly contacting the Clinic Coordinator.

This position may involve more than one clinic visit per day, depending on the needs of the facility.
Requirements:

Proven ability to perform all tasks and duties of a Home Health Aide.

Strong command of the English language (spoken, written, and reading comprehension).

Expertise in providing age-appropriate care to residents.

Able to make independent decisions when needed and work well with staff, residents, families, and external partners.

Excellent communication skills, particularly when conveying information about a resident’s condition.

Must be punctual and committed to maintaining perfect attendance.
