Pay Range: $90,000- $100,682/year

Responsible for supervision of Assisted Living Memory Care staff, program management, staff scheduling, recruiting and training, and all aspects of resident care.

ESSENTIAL FUNCTIONS

  • Oversee operations of the Traditions Memory Care Program, and serve as direct supervisor for staff.
  • Work as a member of the management team
  • Oversee the provision of excellent ADL care, including nutrition and hydration, toileting program, incontinent care, vital signs, activities, and other nursing care. Medication Administration.
  • Ensure staff adapts care giving to build on individual resident strengths and abilities, using and modeling appropriate behavioral intervention techniques with residents.
  • Complete Service Plans on the established basis, partnering with the multidisciplinary care team in Memory Care for input.
  • Communicate with families regarding resident care issues and needs.
  • Verifies all required documentation is in place.
  • Create Staffing Schedule and Staff Assignments.
  • Recruiting, selection and training of staff members.
  • Responsible for writing staff’s Annual Performance Evaluations.
  • Accomplishes staff job results by coaching, counseling, and discipling employees.
  • Maintain a safe working environment by reporting WC work injuries in a timely manner.
  • Enters and approves schedules in Kronos system for direct care staff.
  • Responsible for ensuring budget compliance.
  • Maintain licensing compliance with documentation / binders / forms.
  • Maintains updated ambulance packets, forms, and other supplies.
  • Works with staff to ensure timely food orders and kitchen inventory.
  • Orders routine supplies and maintains inventory.
  • Trains new employees and acts as a resource for team members.
  • Identifies any program inefficiencies and works to correct them and streamline processes.
  • Documents resident care, activities, behavior, and special needs as required by policy and procedure.
  • Provide oversight and direct supervision of care staff and processes.
  • Coordinates with dietary, environmental services, and other departments as needed.
  • Ensures the environment is hygienic and safe.
  • Communicates and cooperates with co-workers and residents. Establishes good working relationships with all departments. Practices good teamwork.
  • Meets with staff regularly and leads team meetings.
  • Conduct assessments in conjunction with Memory Care Lead LVN for new or potential residents.
  • Other duties as assigned.

Requirements

SKILLS, ABILITIES AND EXPERIENCE

  • Experience in geriatrics and caring for residents with dementia.
  • Excellent customer service and organizational skills
  • Sufficient verbal and written communication skills to ensure clear communication with residents, staff and for documentation as required.

EDUCATIONAL REQUIREMENTS/LICENSES/CERTIFICATES

Any combination of education and experience that would provide the necessary knowledge and abilities listed, typically:

  • College degree
  • RCFE Administrator Certification
  • At least 1 year of management experience in healthcare or other life experience in lieu of this.
  • At least 3 years experience in Memory Care or other professional education or certifications in lieu of this.
  • Effective January 2008 and in compliance with the state regulations the SCU Certified Nursing assisntance must complete medication assistance training and pass the competency exam wiht a minimum passing score of 85%

OBRA & TITLE 22 REQUIREMENTS

Ensures that residents' rights to fair and equitable treatment, self determination, individuality, privacy, property and civil rights, including the right to wage a complaint, are well established and maintained at all times.

Maintains confidentiality of appropriate resident care information to assure their rights are protected.

Reports all incidents/accidents, unsafe and hazardous conditions/equipment immediately. Follows established safety rules and regulations. Maintains work area in a clean, orderly and safe manner.
