Key Responsibilities\n

- Establish and administer individualized treatment plans with specific therapy goals based on patient capacity and tolerance, under physician direction.
- Assess patient needs and plan, evaluate, and modify care to meet physical therapy intervention goals.
- Collaborate with interdisciplinary team members to plan and evaluate team goals for each patient.
- Document and record patient and resident condition and progress in accordance with state and federal regulations.
- Educate patients and families on appropriate therapy methods and techniques.
- Communicate patient status and needs to patients, families, and other involved healthcare professionals.

Required Qualifications\n
- \n
- Bachelor's, Master's, or Doctorate Degree from an accredited Physical Therapy program.
- Current Physical Therapist license in the state of employment.
- Current CPR certification.

Preferred Experience / Skills\n

- Minimum of six months of experience in a long-term care or skilled nursing facility environment.
- Demonstrated ability to set realistic and achievable goals for patients and residents in a SNF setting.
