Job Description
The Monday - Friday 3–11 LPN at The Madison Health and Rehab provides compassionate, quality care to long-term care residents and short-term rehabilitation patients. Reporting to the Director of Nursing, Assistant Director of Nursing, and Charge Nurse, this role supports patient health, safety, comfort, and recovery through skilled nursing care and effective collaboration.
 
Pay is $25.50 per hour plus a $2.00 shift differential

Responsibilities
<ul>
<li>Conduct patient assessments and monitor changes in condition.</li>
<li>Administer medications safely and accurately.</li>
<li>Provide wound care according to established care plans.</li>
<li>Measure and monitor vital signs.</li>
<li>Complete timely and accurate patient documentation.</li>
<li>Educate patients on care, treatment, and health management.</li>

Preferred Qualifications
<ul>
<li>Valid Licensed Practical Nurse (LPN) license.</li>
<li>Experience with medication administration and wound care.</li>
<li>Proficiency with electronic health records.</li>
<li>Knowledge of patient assessment and infection control practices.</li>
<li>Strong clinical documentation skills.</li>
<li>Effective communication and teamwork abilities.</li>
