About the Role:

As a Nurse Practitioner, you will play a vital role in our team, providing high-quality care to residents in skilled nursing facilities. This hybrid role combines on-site visits with remote work from home, supported by a dedicated scribe team for efficient documentation.
Responsibilities:

Build strong relationships with skilled nursing facilities and maintain a patient-centered approach

Deliver high-quality preventative care to residents, incorporating Remote Patient Monitoring (RPM), Chronic Care Management (CCM), and Collaborative Care Management (CoCM) into our comprehensive program

Collaborate with our experienced clinical team to ensure seamless alignment with facility processes and protocols

Provide exceptional communication, collaboration, and teamwork skills to drive success in this role

Conduct on-site visits and remote work from home, supported by a dedicated scribe team for efficient documentation
Requirements:

Active NP or PA license in the state of practice

1+ year of experience in Geriatric care and documentation review

Excellent communication, collaboration, and teamwork skills

Ability to work effectively in a fast-paced environment
