About the Role
\n
We are seeking a compassionate and dedicated Registered Nurse (RN) to provide community-based healthcare support for individuals transitioning from hospitals and other care settings into independent community living. This role focuses on improving health outcomes, coordinating care, and supporting individuals with complex medical and behavioral health needs.

Key Responsibilities

- Conduct comprehensive physical, cognitive, and psychosocial assessments.
- Develop, implement, and monitor individualized nursing care plans.
- Participate in discharge planning and care transitions.
- Coordinate primary care, specialty care, and community-based services.
- Monitor participant health status and provide ongoing nursing support.
- Perform medication reconciliation and provide medication education.
- Collaborate with interdisciplinary healthcare teams and external providers.
- Connect individuals with community resources and support services.
- Maintain accurate documentation and complete required reports.
- Promote wellness, recovery, and successful community integration.

Qualifications
\n

- Active New York State Registered Nurse (RN) License.
- Minimum one year of nursing experience preferred.
- Experience in behavioral health, community health, case management, home health, or care coordination is a plus.
- Strong clinical assessment and care planning skills.
- Knowledge of psychiatric and medical medications.
- Excellent communication, documentation, and teamwork skills.
- Experience working with diverse and underserved populations preferred.
