Home care organization is currently seeking a Supervisor to join their team.
Qualifications:
Bachelor's Degree in a health/human services or related field or License as a Registered Nurse, with at least 2 years of case management or other relevant experience; or a Masters with one year relevant experience. Experience working with human service systems supporting children and families with long term, chronic health conditions or developmental disabilities.
Job Responsibilities Customer Service
o Coordinate service for 15 to 20 people and conduct monthly visits
o Compile documentation (IEP, psych, social, medical, etc.) necessary to coordinate enrollment and secure services on behalf of the client, maintaining ongoing Medicaid, program and service eligibility
o Identify service needs, issues and concerns and proactively follow-up
o Collaborate with families, professionals and other involved parties to develop a comprehensive plan of care, completing ongoing timely Reassessments
o Communicate in writing or by telephone with providers and families to resolve issues, secure services or advocate for change
o Return telephone calls and emails promptly
o Visit with client and or guardian (care givers) in the program as prescribed by programmatic regulation or at the request of the family
o Advocate for services as requested by family
o Establish availability to accommodate the families
o Acquire knowledge and recall of families being served
o Develop well written, comprehensive case notes and service plans
o Act as liaison between client/family & Medicaid, LDSS, SSA, school, providers, etc.
Record Management/Regulatory Compliance
o Demonstrate command of regulatory requirements through timely and accurate completion of required paperwork
o Maintain neat and presentable case records for each person served
o File essential documents according to table of contents
o Maintain ongoing case note entries for each event, conversation or meeting that occurs on behalf of a client. Ensure all billing standards are met.
o Draft updated plan documents at least every 6 months for each client with more frequent updates as needed. Complete comprehensive assessments.
Internal Reporting
o Submit accurate and complete billing sheets, disposition logs, site visit schedules, rental requests, and expense reports to your supervisor on time
o Complete time sheets accurately based on time of arrival, departure, lunch break and site visit leave
o Complete time-off requests in advance to ensure prior supervisory approval
o Submit billing to Supervisor & Director of Programs 2x a month for each client
Administrative
o Understand and follow all SKIP and program-related policies and procedures.
o Demonstrate strong written and verbal communication skills.
o Team-oriented and able to work well other team members and with various other internal and external personnel.
o Attend regular staff meetings and participate in bi-weekly supervision
o Attend external training opportunities as scheduled
o Ensure all work efforts and activities completed on behalf of families are case noted, entered in electronic record system and filed daily
o Achieve satisfactory billing/documentation and overall program compliance
o Maintain audit ready, neatly kept records
o Complete appropriate reports for Supervisors, Director and others as requested
o Report changes of address, phone number or program designation updates
o Demonstrate knowledge of all SKIP services and programs
o Demonstrate knowledge of programs and services available to assist population
o Maintain CANS-NY certification.
o Satisfy ongoing training requirements. Effectively work within Health Commerce System, Uniform Assessment System (UAS), MAPP-Health Home Tracking System, Health Home Care Management electronic health record systems, using various other health information technology as required. Complete additional CAH, Health Home and agency-specific trainings as needed.
o Ensure client confidentiality and privacy is maintained.
