A multi-specialty outpatient clinic in Monroe, NY is seeking an experienced&nbsp;Wound Care Nurse Practitioner or Physician Assistant&nbsp;to join our team. We are looking for a provider with strong wound care experience who is committed to delivering excellent patient outcomes. 
&nbsp; 
Responsibilities 
<ul>
<li>Assess, diagnose, and manage acute and chronic wounds </li>
<li>Develop and implement wound care treatment plans </li>
<li>Perform wound evaluations and follow-up care </li>
<li>Educate patients and families on wound management and healing </li>
<li>Document care accurately and efficiently </li>
<li>Work collaboratively with physicians and clinical staff </li>
</ul>
&nbsp; 
Qualifications 
<ul>
<li>Current and unrestricted NP or PA license in New York </li>
<li>Wound care experience required </li>
<li>Strong clinical assessment and communication skills </li>
<li>Ability to work independently in an outpatient clinic setting </li>
