Social Worker
Location: The Woodlands, TX
Pay Rate: Starting at $38.00/hour
Schedule: Monday-Friday | 7:30 AM - 4:00 PM
Dean's Professional Services is seeking an experienced, compassionate, and highly skilled Social Worker to support a leading healthcare system in The Woodlands, TX. This is an excellent opportunity for a licensed social work professional with hospital and/or behavioral health experience who is passionate about helping patients and families navigate complex psychosocial needs, access community resources, and achieve successful care and discharge outcomes.
Responsibilities
- Conduct comprehensive psychosocial assessments to identify patient and family needs, risks, barriers, and available support systems.
- Develop, implement, monitor, and evaluate individualized plans of care in collaboration with patients, families, caregivers, and the interdisciplinary healthcare team.
- Identify and coordinate appropriate community resources, social programs, financial assistance, and support services.
- Provide psychosocial support and intervention related to medical conditions, hospitalization, behavioral health needs, family dynamics, and discharge planning.
- Assist patients and families in navigating complex social, emotional, financial, and environmental challenges that may impact care.
- Collaborate closely with physicians, nurses, case management, behavioral health professionals, and other members of the interdisciplinary team.
- Communicate recommendations and updates effectively while supporting a family-centered approach to care.
- Assist with identifying and addressing barriers to treatment, discharge, and continuity of care.
- Provide support to patients and families experiencing grief, bereavement, end-of-life decisions, and other emotionally complex situations.
- Support families involved in legally or socially complex cases and connect them with appropriate community and institutional resources.
- Utilize crisis intervention, de-escalation, and problem-solving skills when addressing challenging psychosocial situations.
- Maintain accurate and timely documentation in the medical record.
- Protect patient privacy and maintain confidentiality of protected health information in accordance with HIPAA and organizational policies.
- Participate in interdisciplinary communication, handoffs, and care coordination activities.
- Perform other duties as assigned.
Qualifications
- Master's Degree in Social Work (MSW) required.
- Current Texas social work licensure required: LMSW, LCSW, or LMSW-AP.
- Hospital experience and/or Behavioral Health experience required.
- Experience conducting psychosocial assessments and developing individualized plans of care.
- Strong understanding of psychosocial needs across diverse patient populations.
- Experience coordinating community resources and support services.
- Ability to manage crisis situations and complex psychosocial needs.
- Experience supporting patients and families through grief, bereavement, and end-of-life situations preferred.
- Excellent interpersonal, verbal, and written communication skills.
- Strong professional listening and relationship-building abilities.
- Demonstrated initiative, critical thinking, sound judgment, and problem-solving skills.
- Ability to work independently while collaborating effectively with an interdisciplinary healthcare team.
- Strong organizational skills and attention to detail.
- Ability to maintain confidentiality and handle sensitive information professionally.
