Position Summary
\n
The Evening Clinic Nurse Practitioner provides primary and urgent care services during extended clinic hours (approx. 2:00 PM – 10:00 PM) to improve access to timely care and reduce non-emergent emergency department utilization.
\n
\n
This role focuses on same-day and after-hours care, management of acute and chronic conditions, and coordination of follow-up services to ensure continuity of care. The position is a key component of the organization’s strategy to shift care from reactive emergency services to accessible, community-based primary care.
\n
\n
Primary Responsibilities
\n
\n
1. Direct Patient Care
\n
· Provide comprehensive primary care services, including:
\n
· Acute illness evaluation and treatment
\n
· Chronic disease management
\n
· Preventive care services
\n
· Diagnose, treat, and manage a wide range of patient conditions
\n
· Prescribe medications and therapies in accordance with scope of practice
\n
·
\n
2. Same-Day and After-Hours Access
\n
· Deliver care for walk-in and same-day appointments
\n
· Provide an accessible alternative to emergency department use for non-emergent conditions
\n
· Ensure timely evaluation and treatment of patient needs
\n
·
\n
3. Chronic Disease Management
\n
· Manage patients with:
\n
o Diabetes
\n
o Hypertension
\n
o Cardiovascular conditions
\n
Coordinate care with:
\n
· Transitional Care RN
\n
· Population Health Coordinator
\n
· Registered Dietician
\n
\n
4. Care Coordination and Follow-Up
\n
· Ensure appropriate follow-up for patients seen during evening hours
\n
· Coordinate referrals to:
\n
o Home-based Nurse Practitioner/Physician Assistant (for high-risk patients)
\n
o Primary care providers
\n
o Specialty services as needed
\n
o Support continuity of care across the care team
\n
\n
5. Integration with Care Model
\n
· Collaborate with care coordination staff to:
\n
· Reduce avoidable ED utilization
\n
· Improve access to care
\n
· Support population health initiatives
\n
· Utilize patient data and care plans to guide treatment decisions
\n
\n
6. Documentation and Compliance
\n
· Document all patient encounters in the EHR accurately and timely
\n
· Ensure compliance with organizational policies and regulatory requirements
\n
· Support quality reporting and performance improvement initiatives
\n
\n
Performance Expectations
\n
· Increase same-day and after-hours appointment availability by ≥20%
\n
· Contribute to a 10–15% reduction in non-emergent ED visits by Year 3
\n
· Maintain high patient satisfaction and quality of care
\n
· Ensure timely follow-up and continuity of care for patients seen
\n
\n
Required Qualifications
\n
· Current Nurse Practitioner (NP)/Physician Assistant (PA) license in the State of Texas
\n
· Board certification as a Nurse Practitioner (NP), Adult-Gerontology (NP) and Physician Assistant (PA)
\n
· Prescriptive authority in accordance with Texas regulations
\n
· Minimum of 2–3 years clinical experience preferred
\n
\n

o Primary care or urgent care settings

o Rural or underserved populations
\n
o Chronic disease management
\n
o Familiarity with EHR systems (e.g., CPSI/TruBridge)

Work Schedule

· Primary schedule: 2:00 PM – 10:00 PM

· May include flexibility based on clinic needs

· Clinic-based setting with support from Medical Assistants and reception staff

· Collaborative environment with multidisciplinary care team
\n
