Job Summary
Lily of the Valley is seeking a skilled and compassionate PRN Registered Nurse (RN) to provide high-quality recuperative care or Home Care Nursing on an as-needed basis. The PRN RN will assist patients recovering from hospital stays, ensuring they receive proper care, medication management, and support to promote a smooth transition to independent living. The RN will work under the supervision of a clinician.
Responsibilities
- Provide comprehensive nursing care, including assessments, wound care, and medication administration.
- Highly qualified; can also perform assessments.
- Ability to give injections e.g at-home B12 injections to Long-term, high-needs clients.
- Monitor patients’ progress and respond to changes in their medical conditions.
- Collaborate with physicians, case managers, and support staff to develop personalized care plans.
- Educate patients on self-care, medication adherence, and wellness strategies.
- Ensure compliance with infection control and safety protocols.
- Document all patient interactions, assessments, and interventions accurately in medical records.
- Support patients transitioning to permanent housing, rehabilitation, or other care settings.
- Administer medications and treatments as prescribed, ensuring compliance with HIPAA regulations.
- Maintain a safe and clean environment for patients by adhering to infection control standards.
- Participate in training programs to stay updated on best practices in internal medicine and long-term care.
Qualifications/Experience :
- Active RN license in Minnesota
- Experience, preferably in recuperative care, home health, med-surg, or community health.
- Strong clinical assessment and critical thinking skills.
- Experience working with vulnerable populations, including individuals experiencing homelessness or chronic health conditions, is a plus.
- Ability to work independently while being a collaborative team player.
- Excellent communication and patient education skills.
- Flexibility to work PRN shifts, including weekends and holidays, as needed.
