Licensed Practical Nurse (LPN) – Per Diem Night Shift Hospice
We are seeking experienced Licensed Practical Nurses (LPNs) for per diem night shift hospice assignments throughout the Fort Lauderdale to Boca Raton area.
In this role, you will provide compassionate, high-quality care to patients in their homes in accordance with prescribed medical treatment, agency policies, and individual plans of care. LPNs may perform designated nursing procedures under the supervision of a Registered Nurse.
Pay: $27-29
Shift: 8hrs
Responsibilities:

<li>Provide skilled nursing care in patients' homes</li>
<li>Follow established plans of care and physician orders</li>
<li>Monitor and report patient conditions and changes</li>
<li>Maintain accurate documentation</li>
<li>Communicate effectively with patients, families, and healthcare team members</li>
<li>Perform nursing procedures as authorized under RN supervision</li>
</ul>
Requirements
<ul>
<li>Active Florida LPN license</li>
</ul>
<ul>
<li>Strong Hospice experience required</li>
</ul>
<ul>
<li>Experience with HCHB (Homecare Homebase) required</li>
</ul>
<ul>
<li>Reliable transportation</li>
</ul>
<ul>
<li>All onboarding requirements</li>
</ul>
<ul>
<li>Required online learning modules</li>
</ul>
<ul>
<li>Mandatory 8-hour in-person training</li>
