Job Title: Wellness Navigator
Job Description
As a Wellness Navigator, you will assist providers by guiding patients through community resources, ensuring they have the appropriate forms and documentation. You will build relationships with local schools, churches, nonprofits, and community organizations, and represent our services at health fairs, community events, and partner meetings. Your role will also include conducting presentations and outreach to raise awareness of our medical, dental, behavioral health, and enabling services.
Responsibilities
- Assist providers in guiding patients through community resources.
- Provide necessary forms and documentation to patients.
- Build and maintain relationships with schools, churches, nonprofits, and community organizations.
- Attend and represent the organization at health fairs, community events, and partner meetings.
- Conduct presentations and outreach to raise awareness of our services.
Essential Skills
- Customer service
- Call center experience
- Scheduling appointments
- Healthcare experience
- Medical assisting
- Patient care
- Bilingual in Spanish
- Experience with electronic health records
Additional Skills & Qualifications
- Healthcare experience of at least 6 months
- High school diploma or equivalent
- Medical assisting diploma or license preferred
Work Environment
You will work in a clinic setting as part of a small team of two wellness navigators per location. The role offers opportunities for growth within the organization, with a training program and the potential to transition to a direct position upon successful completion of the duration.

This is a Contract to Hire position based out of Santa Barbara, CA.
Pay and Benefits
The pay range for this position is $25.00 - $25.00/hr.
