We are seeking a dedicated and experienced Physician to provide comprehensive medical care in a Skilled Nursing Facility (SNF) in South Bend, IN. Duties and Responsibilities

Perform comprehensive medical assessments and develop individualized treatment plans

Diagnose, manage, and treat acute and chronic medical conditions

Provide ongoing monitoring and follow-up care for residents

Collaborate with interdisciplinary teams to enhance patient care and quality outcomes

Skills and Qualifications

Active Medical Doctor (MD) or Doctor of Osteopathic Medicine (DO) license in the state of Indiana Board certification or eligibility in Internal Medicine, Family Medicine, or Geriatrics

Preferred Experience in Skilled Nursing Facility (SNF), long-term care, or geriatric medicine preferred
