Interdisciplinary Team Coordinator

The Center for Elders’ Independenceis a PACE (Program of All-Inclusive Care for the elderly) organization that uses an interdisciplinary team approach to care planning and care implementation for the purpose of providing high quality, affordable, integrated health care services to the elderly, including an Adult Day Health Center, and promoting autonomy, quality of life and the ability of individuals to live in their communities.

The Position: Under the direction of the Center Director, the Interdisciplinary Team (IDT) Coordinator is responsible for supporting the IDT by coordinating participant service delivery, ensuring compliance with regulatory requirements and documenting IDT meetings and care planning discussions. The IDT Coordinator also provides administrative support to the Center Director and participates in quality improvement initiatives that impact participant care at the center.

The rate range for the Interdisciplinary Team (IDT) Coordinator position at Center for Elders Independence is $22.50 to $33.76 per hour. Pay rate based on the market for the IDT Coordinator position, as well as experience, skills, abilities and work history.

DUTIES AND RESPONSIBILITIES:

  • Coordinates IDT meetings by preparing agendas, documenting meeting minutes, and managing information for development of the participants care plan.
  • Manages Service Determination Request documentation in the electronic health record (EHR), including writing denial letters, tracking when services are provided and monitoring IDT compliance with regulation.
  • Monitors, tracks and gathers necessary data for completion of required regulatory reporting and submission.
  • Manages grievances and appeals process, including completing investigations and drafting and editing resolution letters in collaboration with the Center Director and other department leaders.
  • Provides administrative support to the Center Director such as scheduling meetings, interviews, tours, etc.
  • Interfaces directly with participants and their caregivers upon identification of being at risk of disenrollment from CEI.
  • Assists with tracking of participant rounding to support satisfaction with CEI services.
  • Collaborates with the clinic team for centralized assessment scheduling.
  • Collaborates with Outreach and Enrollment department to schedule center tours.
  • Reviews participant records as needed in collaboration with the Center Director for both internal and external audits.
  • Support onboarding and training of new IDT members.
  • Maintain the confidentiality of all company procedures, results and information about participants or families.
  • Comply with all CEI Policies and Procedures, training requirements and safety guidelines.
  • Perform other duties as assigned in a positive and helpful manner.

QUALIFICATIONS:

  • Associates degrees with 5 years of experience required, Bachelor’s degree in health or human services or other related field, preferred.
  • Minimum of 2 years’ experience in health care setting such as in quality or compliance.
  • Experience with electronic health records (EHR), preferred.
  • Proficiency in Microsoft Office Suite applications.
  • Excellent organizational and time-management skills.
  • Excellent written and verbal communication skills and demonstrates ability to interact clearly and effectively with both internal and external customers.
  • Skilled in establishing and maintaining effective working relationships with participants, peers and the public.
