Pay Range: $60 - $69/hr

Position Overview

We are seeking an experienced Registered Nurse Case Manager to coordinate patient care, facilitate discharge planning, and support continuity of care in an acute-care hospital.

Required Qualifications

• Active California RN License required.

• Minimum 2 years of acute-care hospital Case Manager experience.

• Hospital experience required.

• Experience with care coordination and discharge planning.

• First-time travelers accepted; clinical screening required.

• Pending California license accepted if active by the start date.

• Availability for weekend rotation and department needs.

• Experience working with approximately 1:20 patient ratios.

• Ability to work in an acute-care facility environment.

Responsibilities

• Provide case management services in an acute-care hospital setting.

• Perform comprehensive patient assessments to identify care coordination needs.

• Coordinate care across interdisciplinary healthcare teams and services.

• Develop and facilitate appropriate discharge plans.

• Coordinate transitions of care and post-acute services.

• Identify barriers to discharge and develop appropriate solutions.

• Collaborate with physicians, nurses, social workers, utilization review teams, and other healthcare professionals.

• Communicate with patients and families regarding care plans, discharge needs, and available resources.

• Monitor patient progress and facilitate timely access to appropriate services.

• Maintain accurate and timely case management documentation.

• Support continuity of care throughout the patient's hospital stay.

• Participate in weekend rotation and meet department staffing needs.

• Follow hospital policies, patient safety standards, and applicable regulatory requirements.
