Xenia Health and Rehab is hiring an Assistant Director of Nursing/MDS Coordinator for our skilled nursing and rehabilitation center at 126 Wilson Drive in Xenia, Ohio. This Assistant Director of Nursing/MDS Coordinator role is a hybrid seat: you'll own the Minimum Data Set (MDS) process for the building and support the Director of Nursing (DON) as Assistant Director of Nursing (ADON). Prior MDS experience and prior skilled nursing facility (SNF) / long-term care experience are both required.

What You'll Do

Complete and coordinate accurate, timely MDS assessments and care plans under the Patient Driven Payment Model (PDPM)

Manage the MDS schedule, Assessment Reference Dates (ARDs), and transmissions

Support the DON with day-to-day clinical operations and nursing quality

Help lead survey readiness and regulatory compliance

Serve as a clinical resource and step in for the DON when needed
Qualifications

Active Ohio Registered Nurse (RN) license

Minimum Data Set (MDS) Coordinator experience

Skilled nursing facility (SNF) / long-term care experience
