Qualifications
Required
- Active Registered Nurse (RN) license in the State of Connecticut
- Current BLS certification
- Minimum of 1 year Hospice nursing experience
Preferred
- Home health experience
- Strong knowledge of symptom management and end-of-life care
What You’ll Do
- Perform comprehensive nursing assessments for hospice patients
- Develop and implement individualized care plans in collaboration with the interdisciplinary team
- Provide symptom management and pain control according to physician orders
- Educate patients and families about hospice care, medications, and comfort measures
- Provide emotional support and guidance to patients and caregivers
- Coordinate care with physicians, social workers, chaplains, and other healthcare professionals
- Monitor patient condition and respond to changes in health status
- Maintain accurate and timely clinical documentation
- Participate in interdisciplinary team meetings and care planning discussions
Compensation
Salary: $100,000 – $115,000 per year
