Adult Community Training (ACT) is seeking a dependable and compassionate Licensed Practical Nurse (LPN) to support four adults in a residential home setting in Lenoir City. This position plays a key role in ensuring the health, safety, and overall well being of the individuals we support while working alongside Direct Support Professionals (DSPs).
Schedule:
Monday through Friday, 4:00 PM to 12:00 AM
Consistent weekday schedule with no rotating shifts
What You’ll Do:

Provide direct nursing care and oversight for four individuals in a home setting

Administer medications and treatments as prescribed

Monitor health conditions and respond to changes or concerns

Complete nursing documentation accurately and in a timely manner

Communicate effectively with DSP staff, supervisors, physicians, and other members of the care team

Provide guidance and support to DSPs related to medical needs and care plans

Assist with appointments, follow ups, and coordination of care as needed

Ensure compliance with all state regulations and ACT policies
What to Expect in This Role:

This is a hands on role in a home environment, not a clinical or hospital setting

You will be working closely with the same individuals each day, building consistency and familiarity

The pace can vary depending on the needs of the individuals, including times that require quick response and critical thinking

Teamwork with DSPs is essential, as they provide daily support and rely on nursing direction for medical care
Qualifications:

Current LPN license in the state of Tennessee

Experience working with individuals with intellectual or developmental disabilities is preferred, but not required

Strong communication and documentation skills

Ability to work independently while also being part of a team

Reliable and consistent attendance
