The QMHP/CSS provides client assessment and monitoring and community support/case management services to clients enrolled in NCMMHC services.
B. DUTIES
1. Provide a minimum of 15 face-to-face client contacts per week.
2. Complete and sign documentation or related paperwork contemporaneously with service provision (within three (3) business days) and in accordance with agency and certification standards.
3. Participate in mobile crisis team coverage when assigned.
4. Provide ongoing assessment of client status including personal strengths, progress, problems and needs.
5. Participate in client staffing, assessment and treatment planning.
6. Promotes measurable improvement in mental/medical health outcomes.
7. Communicate information about the client to all key staff who provide services to the client.
8. Complete continuing education hours as required by DMH and agency certification standards.
9. Conduct informational programs for civic and social groups as requested.
10. Assist in training as requested.
11. Participate in quality assurance activities as requested.
12. Intercede, with appropriate authorization, on behalf of individual clients within the community-at-large to assist the client in achieving and maintaining their community adjustment.
13. Maintain contact with clients who are hospitalized, and participate in and help facilitate discharge planning.
14. Provide individualized assistance in creating a personal support system that includes work with family members, legal guardians, or significant others regarding the needs and abilities of an identified client.
15. Cooperate with PSR and other agency group activities so as to incorporate the groups into the overall rehabilitative system of Community Psychiatric Rehabilitation programs.
16. Maintain current CPR certification and First Aid certification.
17. Maintain client confidentiality.
18. Perform related work as requested to carry out the purposes, goals and objectives of the agency.
QUALIFICATIONS
The QMHP must be one of the following as listed in the DMH code 9CSR 30-4.030: A physician licensed under Missouri Law to practice medicine or osteopathy, a psychiatrist licensed under Missouri law, a psychologist licensed under Missouri law, a professional counselor licensed under Missouri law, a clinical social worker licensed under Missouri law, a psychiatric nurse, a registered professional nurse, an individual possessing a master’s or doctorate degree in a related field, an occupational therapist, an advanced practice nurse, or a psychiatric pharmacist. Demonstrate acceptable documentation skills.
