Registered Nurse – Rehab travel assignment in a skilled nursing facility setting based in Laconia, New Hampshire. This 14-week assignment begins 11/29/2026 and offers a weekly pay range of $1,418 to $1,500 with guaranteed hours of 36 per week. Work 3x12-hour night shifts from 19:00 to 07:30. This assignment takes place in a skilled nursing facility/LTC nursing home and is managed by Healthforce Healthcare LLC.

Responsibilities
- Administer medications and treatments per the plan of care and document responses in the electronic record.
- Implement rehabilitation protocols and therapies in collaboration with physical, occupational, and speech therapy to promote patient mobility and independence.
- Monitor patients’ conditions, perform regular assessments, and communicate changes to the interdisciplinary team.
- Ensure safety, infection control, wound care where applicable, and fall risk mitigation on all shifts.
- Provide education to patients and families about mobilization strategies, activity programs, and progress toward goals.
- Document thoroughly and maintain clear, concise communication within the care team to support coordinated, high-quality care.
Requirements
- Current RN license (state-specific) and eligibility to practice in New Hampshire or as required for the assignment.
- Relevant experience in rehabilitation or skilled nursing facility settings preferred.
- Ability to work 3x12-hour night shifts and adhere to the stated schedule.
- Strong assessment, observation, documentation, and communication skills.
- Demonstrated ability to collaborate with an interdisciplinary team and support care plan updates.
