Job Title: Hospice CNA | Every Visit Makes a Difference

Compensation: $19-$25/hour DOE + mileage reimbursement paid by wage + $500 Sign-On Bonus

Benefits: Full benefits package available for full-time staff

Schedule: Per Diem or Full-Time | Weekend rotation

Location: 21-County Twin Cities Service Area

What You'll Do

• Provide hands-on assistance with bathing, dressing, grooming, toileting, and mobility.
  
• Support patient comfort while observing and reporting changes in condition.
  
• Assist with care needs as directed by the interdisciplinary team.
  
• Provide companionship and emotional support to patients and families.
  
• Communicate effectively with nurses and other members of the hospice team.
  
• Help maintain a safe, clean, and comfortable environment.
  
• Deliver care that reflects each patient's individualized plan of care.
  

Who We're Looking For

• Current Minnesota CNA certification.
  
• Hospice experience preferred, with palliative care, oncology, home care, long-term care, or assisted living experience also welcomed.
  
• Compassionate, dependable, and patient-focused.
  
• Strong communication and teamwork skills.
  
• Valid driver's license and reliable transportation.
