Job Description Mental Health Advocate – Community-Based ACT Services

Segue, Inc. is looking for a compassionate, dependable Mental Health Advocate to join our Assertive Community Treatment (ACT) team.
This isn't a traditional office-based mental health position. You'll work directly with individuals in their homes and throughout the community, helping them build skills, overcome barriers, connect with resources, navigate challenges, and work toward greater independence.
You'll also be part of a multidisciplinary team, working alongside other behavioral health professionals to provide coordinated, person-centered support to individuals with serious mental illness and complex needs.
This position is eligible for a $4,000 retention bonus, paid according to established retention bonus terms.

Segue, Inc. is a nonprofit behavioral health organization providing community-based mental health services in Jackson and Hillsdale Counties. As a provider within the local community mental health (CMH) system, Segue partners with LifeWays and other local agencies to provide services that support recovery, independence, and quality of life for individuals in our communities.

What You'll Do
- Build supportive, professional relationships with individuals receiving ACT services.
- Meet with consumers in their homes, at Segue, and throughout the community.
- Help develop and carry out individualized, person-centered treatment plans.
- Support consumers in developing independent living and community-integration skills.
- Develop and lead groups and activities that help consumers build skills, connect with others, and support their recovery and wellness goals.
- Help individuals navigate housing, benefits, employment, transportation, healthcare, and other community resources.
- Provide advocacy, crisis support, referrals, and linkage to needed services.
- Coordinate care with hospitals, healthcare providers, community agencies, and other members of the consumer's support network.
- Support medication-related needs within your training and role.
- Provide or coordinate transportation when needed.
- Complete required assessments, progress notes, and other documentation in our electronic medical record.
- Collaborate closely with other members of the multidisciplinary ACT team.
These responsibilities reflect the community-based assessment, treatment, advocacy, care coordination, crisis intervention, skills development, and resource-support functions already established in the position.
What We're Looking For
Required:
- Bachelor's degree in Social Work, Human Services, Psychology, Counseling, Criminal Justice, Nursing, Education, or a related field.
- Qualified Mental Health Professional (QMHP) or ability to meet applicable QMHP requirements.
- Valid Michigan driver's license, acceptable driving record, and required automobile insurance.
- Comfortable working independently in consumer homes and community settings.
- Strong communication, organization, interpersonal, and documentation skills.
- Ability to remain calm, professional, and solution-focused in challenging situations.
- Ability to participate in required on-call responsibilities.
The breadth of qualifying bachelor's-degree fields is consistent with the current JD and is something I'd make very visible rather than inadvertently making candidates think they need a social-work degree.
We'd Especially Like to Meet You If...
You have experience in community mental health (CMH), case management, ACT, behavioral health, crisis services, social services, group facilitation, or other community-based human services.
Experience supporting individuals with serious mental illness, person-centered planning, Motivational Interviewing, trauma-informed care, recovery-oriented services, or electronic medical records is helpful—but you don't need to come to us already knowing everything.
We're looking for someone who can connect with people, work independently, collaborate with a team, exercise good judgment, and genuinely wants to help individuals succeed in their communities.
Schedule & Compensation
Schedule: Full-Time | Monday-Friday | 8:00 am - 4:30 pm
Pay: Starting at $20.00/hour
Retention Bonus: $4,000, paid according to established retention bonus terms
This position includes community-based work, local travel, consumer transportation, and participation in an on-call rotation. Those expectations are consistent with the existing working-environment requirements for the role.
