Solace Hospice & Palliative Care Inc is looking for a compassionate and dedicated Hospice Certified Nursing Assistant to join our caring team in Saint Charles, IL. In this meaningful role, you'll provide hands-on personal care and emotional support to patients and families during life's most important moments. Responsibilities:
- Provide personal care services including bathing, grooming, oral hygiene, and repositioning for hospice patients
- Assist with activities of daily living (ADLs) in patient homes or care facilities
- Monitor and report changes in patient condition, vital signs, and comfort levels to the nursing team
- Offer compassionate emotional support and companionship to patients and their families
- Assist with light housekeeping, meal preparation, and personal laundry related to patient care
- Document patient care activities accurately and in a timely manner
- Collaborate with the interdisciplinary hospice care team to ensure a holistic patient experience
Requirements:
- Current CNA certification in the state of Illinois
- Prior hospice, palliative care, or end-of-life care experience preferred
- Strong understanding of comfort-focused, patient-centered care principles
- Excellent communication and interpersonal skills with patients, families, and care teams
- Reliable transportation and valid driver's license for home visit travel in the Saint Charles, IL area
- Ability to work independently with compassion, patience, and professionalism
- CPR certification and ability to meet health screening requirements
