• Gross Pay: $2,850/week
• W2 Pay: $23/hour + $1,930/week non-taxable stipend

• Experience: Minimum 2 years of acute care hospital Case Manager experience
• Requirements: Care Coordination and Discharge Planning experience
• Local Candidates: Candidates within 50 miles are accepted. Pay may vary for local candidates.

\n • First-timers accepted: Yes – Clinical will screen 1st-time travelers.

• Acute hospital case management / utilization review experience • Patient assessment and care coordination • Discharge planning and plan of care development • Admission criteria / InterQual utilization review • Experience coordinating DME, SNF placement, and ambulance transportation independently. • Knowledge of drug/alcohol abuse treatment options, patients experiencing homelessness, patients with mental health history or in crisis, advance directives, POLST, protective services process, etc. • Ability to organize and participate in family meetings. • Participate in the development of the treatment plan.
