Responsibilities:

Provide high-quality, patient-centered care to seniors and adults in their homes, including assistance with daily living activities such as bathing, dressing, and grooming

Perform essential care tasks as assigned, including medication management, wound care, and mobility assistance

Maintain accurate and detailed records of client care and services provided

Collaborate with our interdisciplinary team to develop and implement individualized care plans

Maintain confidentiality and adhere to all HIPAA guidelines
