Hospital Liaison for Skilled Nursing Position Overview

Avina Healthcare is seeking an experienced Hospital Liaison to support referral development and admissions in the Weyauwega area. This is a hospital-based position, requiring regular travel to assigned hospital accounts rather than working from a single facility.

Hospital Liaison for Skilled Nursing Key Responsibilities

• Develop and maintain relationships with hospital case managers, social workers, discharge planners, and other referral sources.

• Meet with patients and families to explain skilled nursing and rehabilitation services and assist with the transition of care.

• Coordinate referral reviews with Central Intake and facility teams to determine clinical and financial eligibility.

• Follow referrals through admission, communicate updates, and help resolve admission barriers.

• Maintain accurate, timely documentation in ExaCare.

• Support referral growth and meet established admission goals.

Hospital Liaison for Skilled Nursing Qualifications

• Previous experience as a Hospital Liaison or in a comparable skilled nursing admissions or healthcare business development role.

• Knowledge of hospital discharge planning, skilled nursing services, and Medicare, Medicaid, and managed care payer requirements.

• Strong communication, relationship-building, organizational, and follow-through skills.

• Ability to work independently and manage multiple hospital accounts.

• Valid driver’s license and ability to travel regularly throughout the assigned territory.
