Hybrid (Remote + Field)
- Candidates must reside in Homestead / Florida City area (33033, 33032)
Pay: From $34.14 per hour
Schedule:
Monday–Friday | 8:00 AM – 5:00 PM
No weekends No holidays
Position Overview
We are seeking a Social Work / Human Services Case Manager to support Medicaid Long-Term Care (LTC) and Comprehensive Program enrollees. This is a hybrid role, combining remote documentation with field-based member visits.
- Candidates must reside in Homestead / Florida City area (33033, 33032) area and be willing to travel.
Key Responsibilities
- Coordinate case management services for Medicaid Long-Term Care members
- Conduct comprehensive member assessments using care management tools and data review
- Develop, implement, and monitor individualized care plans
- Collaborate with interdisciplinary care teams to achieve optimal health outcomes
- Track progress and update documentation in accordance with program requirements
Travel Requirements
- Up to 75% field-based travel within Miami-Dade County
- Travel time paid at hourly wage
- Mileage reimbursed at IRS rate
- Reliable transportation required
Required Qualifications
- Case management experience required
- Bachelor’s degree in Social Work or a related Human Services field
- Candidates must reside in Homestead / Florida City area (33033, 33032)
- Bilingual Spanish/English required
- Ability to work independently in the field and remotely
- Authorized to work in the U.S.
Preferred Qualifications
- Experience in Long-Term Care (LTC) or Medicaid programs
- Strong organizational, time-management, and multitasking skills
- Excellent verbal and written communication abilities
- Comfortable working in a fast-paced, changing environment
Training
- 2 weeks of remote training
- Monday–Friday | 8:00 AM – 5:00 PM
- Full attendance required
