Perform complete physical assessment of patients
\n
Assist with supervision of care to re-evaluate the patients condition
\n
Insure that skilled nursing care is provided
\n
Initiate preventive and rehabilitative nursing procedures
\n
Observe the patient and report any changes to the provider
\n
Teach and counsel patient and family members
\n
Participate in patient centered care planning conferences
\n
Complete all documentation in a timely manner
\n
\n
BGD: RN, State of NY
\n
Must have 1-2 years of RN experience
\n
Licensed Home Care experience is preferred
\n
\n
