$5,000 Signing BonusEpiphany Family Services is offering a $5,000 signing bonus, payable in installments according to company policy. Bonus eligibility is contingent upon continued employment and satisfactory performance.
About the RoleEpiphany Family Services is seeking an experienced Tailored Care Manager Supervisor to lead a team of Care Managers providing whole-person, integrated care to individuals with complex behavioral health and medical needs.This position supports individuals with Intellectual/Developmental Disabilities (I/DD), Traumatic Brain Injury (TBI), serious mental illness (SMI), and substance use disorders (SUD).The supervisor will ensure high-quality care management services, regulatory compliance, and strong collaboration with healthcare providers, community partners, and care teams.This role operates in a hybrid work environment, including office collaboration, community field visits, and remote work from home.
ResponsibilitiesLeadership & Supervision
- Supervise, mentor, and support a team of Tailored Care Managers
- Provide coaching, supervision, and performance management
- Ensure staff meet productivity and documentation standards
Care Management Oversight
- Oversee whole-person care planning and coordination
- Monitor caseload assignments and service delivery
- Ensure care gaps are addressed and members receive appropriate services
Compliance & Quality
- Ensure compliance with NC Medicaid Tailored Plan requirements
- Conduct case record reviews and quality assurance monitoring
- Maintain compliance with 10A NCAC 27G standards
Collaboration
- Work closely with healthcare providers, hospitals, and community partners
- Support integrated care teams to improve member outcomes
- Identify service gaps and improve service delivery
Required QualificationsCandidates must meet Qualified Professional (QP) requirements under 10A NCAC 27G regulations
Education & Credential RequirementsApplicants must meet one of the following:
- Licensed or provisionally licensed clinician (LCSW, LCMHC, LCAS, RN, etc.)
- Registered Nurse licensed in North Carolina with 4 years MH/DD/SUD experience
- Master’s degree in a human services field + 1-year supervised MH/DD/SUD experience
- Bachelor’s degree in the human services field + 2 years’ supervised MH/DD/SUD experience
- Bachelor’s degree in another field + 4 years’ supervised MH/DD/SUD experience
- Fully certified NCASPPB professional with 4 years MH/DD/SUD experience
Note: CADC-R and CADC-I certifications alone do not meet QP requirements without the required degree and experience.
Experience Requirements
- Minimum 2+ years experience in Tailored Care Management (TCM)
- Minimum 3 years supervisory or leadership experience
- Experience working with individuals with:
- I/DD
- TBI
- Serious Mental Illness
- Substance Use Disorders
Preferred Qualifications
- Experience working within NC Medicaid Tailored Plans
- Experience with Electronic Health Records (EHR)
- Knowledge of DSM-5
- Experience supervising behavioral health or care management teams
