About the Role:

The Aftercare Worker plays a crucial role in supporting individuals who have recently transitioned from care or rehabilitation settings back into their communities. This position focuses on providing emotional, practical, and social support to help clients adjust and thrive in their new environments. The Aftercare Worker will work closely with clients to develop personalized aftercare plans that address their unique needs and goals. By fostering a trusting relationship, the Aftercare Worker will empower clients to build independence and resilience. Ultimately, the major end result of this role is to enhance the quality of life for clients and facilitate their successful reintegration into society.

Minimum Qualifications:

  • Highschool Diploma
  • Experience working with individuals in a care or rehabilitation setting.

Preferred Qualifications:

DCF 40 HOURS FIRST 90 DAYS.

  • Experience in case management or community outreach.

Responsibilities:

  • Conduct regular check-ins with clients to assess their well-being and progress towards their aftercare goals.
  • Develop and implement individualized aftercare plans in collaboration with clients and other support services.
  • Provide emotional support and guidance to clients, helping them navigate challenges and celebrate successes.
  • Connect clients with community resources, such as housing, employment, and mental health services.
  • Maintain accurate and confidential records of client interactions and progress, ensuring compliance with relevant regulations.

An aftercare worker provides supervision and support to children outside of regular school hours. They ensure a safe and engaging environment, offering emotional support and assisting with activities like homework or recovery needs. Common duties include supervising children, planning activities, maintaining a safe environment, and communicating with parents.

Skills:

The required skills for this role include strong communication and interpersonal abilities, which are essential for building rapport with clients and understanding their needs. Organizational skills are vital for managing multiple client cases and ensuring that all documentation is accurate and up-to-date. Problem-solving skills will be utilized daily as the Aftercare Worker helps clients navigate various challenges they may face during their transition. Preferred skills, such as knowledge of community resources, will enhance the Aftercare Worker’s ability to connect clients with appropriate support services. Additionally, empathy and active listening skills are crucial for providing the emotional support that clients need to feel understood and valued.
