- Pay: $24.50–$32.77/hr. (Based on experience) + Benefits
Position Overview
We are seeking a Licensed Practical Nurse (LPN) to join a collaborative Primary Care outpatient clinic in New Hope, Minnesota.
The LPN will provide clinical support to providers and the healthcare team while helping deliver safe, efficient, and patient-centered care to patients across the lifespan.
This is an excellent opportunity for an LPN with previous clinical experience and Primary Care/outpatient experience who enjoys working directly with patients and collaborating with an interdisciplinary healthcare team.
-> Key Responsibilities
- Provide nursing care and clinical support within the LPN scope of practice.
- Assist providers with patient examinations, treatments, and procedures.
- Obtain vital signs and complete patient assessments as directed.
- Administer medications, injections, and immunizations according to provider orders and organizational protocols.
- Collect specimens and perform laboratory testing as appropriate.
- Monitor patients for changes in condition and communicate concerns promptly to providers and supervising nurses.
- Support patient flow and efficient clinic operations.
- Provide patient and family education regarding medications, treatments, follow-up care, and health management.
- Assist with prescription refill requests and patient follow-up.
- Provide telephone and clinical support to patients as assigned.
- Assist with care coordination and communication between patients, providers, and other healthcare services.
- Support prior authorization activities and other clinical/administrative workflows.
- Maintain accurate and timely documentation in the electronic medical record.
- Follow infection-control, patient-safety, HIPAA, and organizational policies.
- Collaborate with physicians, nurses, Medical Assistants, and other healthcare professionals to provide high-quality patient care.
-> Required Qualifications
- Active Minnesota LPN/Practical Nurse license required at time of submission.
- Previous experience working in a clinical healthcare setting required.
- Current BLS certification required.
- Strong clinical judgment and communication skills.
- Ability to work collaboratively in a fast-paced outpatient environment.
- Ability to provide safe, compassionate care to patients across different age groups.

- Previous Primary Care or outpatient clinic experience.
- Experience with patient triage and telephone support.
- Experience administering injections and immunizations.
- Phlebotomy and laboratory testing experience.
- Experience with chronic disease management and patient education.
- Experience with prescription refills and prior authorizations.
- EHR/EMR experience.
- Experience caring for pediatric, adult, and geriatric populations.
