Position Summary
As a 1915(i) Clinical Team Specialist, you will play a vital role in managing assessments, service planning, and authorization requests for clients enrolled in NC Medicaid 1915(i) Home and Community-Based Services. Your work will directly contribute to the well-being and success of the individuals we serve.
Key Responsibilities
- Conduct independent eligibility assessments and develop independent risk plans for clients.
- Coordinate care with LME/MCO care managers and submit service authorization requests.
- Monitor service delivery to ensure compliance with 1915(i) state plan guidelines.
- Maintain accurate records and documentation to support client care and program requirements.
Required Skills and Qualifications
- NC Qualified Professional (QP) status is required.
- At least 1 year of dedicated experience with NC Medicaid 1915(i) services or Medicaid waiver programs.
- Strong organizational and communication skills to effectively collaborate with care managers and clients.
- A commitment to upholding the highest standards of care and compliance.

- Locations: Multiple locations across North Carolina, including Charlotte, Concord, Salisbury, Kannapolis, Greensboro, Asheville, Rutherfordton, Hickory, Forest City, Gastonia, Shelby, Kings Mountain, Monroe, and High Point.
