🏠 Travel Home Health Registered Nurse (RN)
\n 📍 Location: Newport, Vermont
\n 📅 Contract: 25 Weeks
\n ☀️ Schedule: Monday–Friday | 8:00 AM–5:00 PM
\n ⏱️ Hours: 32 Hours/Week
\n 💻 EMR: HealthWyse (Careficient)
\n We’re seeking an experienced Home Health RN for a 25-week travel assignment providing skilled care to patients in their homes. This role requires strong OASIS expertise, Home Health/Hospice experience, and the ability to travel independently between patient visits.
\n ✅ Requirements
\n Active Vermont RN License
\n Minimum 3 years of Home Health nursing experience
\n OASIS experience required
\n Home Health and Hospice experience required
\n Experience traveling from patient to patient for home visits
\n Current BLS/BCLS certification
\n Valid driver’s license in good standing
\n Ability to adapt to changing patient schedules and daily assignments
\n 🩺 Key Responsibilities
\n Conduct comprehensive home health patient assessments
\n Complete accurate OASIS assessments and documentation
\n Provide skilled nursing care during home visits
\n Develop and adjust patient care plans as needed
\n Coordinate care with physicians and interdisciplinary teams
\n Monitor patient progress and communicate changes in condition
\n Maintain accurate clinical documentation
\n 📌 Assignment Highlights
\n 32 hours per week
\n Some weekend coverage required
\n Emergency-only call may be scheduled
\n Holiday coverage required
