Pay Rate: $60–$65/hour

Position Type: Contract

Schedule:

• Monday: 3:00 PM – 12:00 AM
• Thursday: 2:00 PM – 12:00 AM
• Friday: 2:00 PM – 12:00 AM
• Saturday: 2:00 PM – 12:00 AM
• 35 Hours Per Week
• Weekends and holidays included

Position Summary

We are seeking an experienced Licensed Master Social Worker (LMSW) to provide psychosocial support and social work services in a healthcare setting. The LMSW will assess patient needs, provide supportive therapy, assist with discharge planning, and coordinate appropriate referrals and resources.

Key Responsibilities

• Conduct comprehensive psychosocial assessments of patients.
• Provide supportive therapy and counseling based on patient needs.
• Assist patients and families with social and emotional concerns.
• Support discharge planning and coordinate appropriate referrals.
• Connect patients with appropriate community and healthcare resources.
• Maintain accurate and timely documentation of social work services.
• Collaborate with interdisciplinary healthcare teams to support patient care.
• Maintain confidentiality and follow applicable policies and professional standards.

Required QualificationsEducation

• Master’s Degree in Social Work (MSW) required.

Licensure & Certification

• Valid Licensed Master of Social Work (LMSW) license issued by the New York State Education Department (NYSED) required.
• Primary Source Verification of licensure required.

Experience

• Minimum 1 year of LMSW experience required.
• Experience conducting psychosocial assessments and supporting discharge planning preferred.
