Provides Wellness programming to clients to promote functional independence and decrease the detrimental effects of inactivity. Specific responsibilities include documentation, communication, class instruction and fitness screening.
This position requires a flexible schedule to meet the needs of the property. This may include evenings and weekends. This position is a non-exempt hourly position.

Position Responsibilities:
· Maintains good judgment in difficult situations, asks appropriate questions when unsure of assignments, and takes initiative in areas expected of him/her. Exhibits ability to work alone, without supervision.
· Conveys feedback from patient and/or family to therapy and nursing personnel as needed in coordinating the total rehabilitation program of the client.
· Participates in client, family, and staff education.
· Maintains positive relationships and rapport with co-workers, clients, family members, and facility personnel, modeling respectful behavior by exhibiting professionalism with coworkers.
· Maintains knowledge and adheres to all applicable and established confidentiality standards including all HIPAA rules and regulations.
· Reports, as appropriate, actual or suspicious activity related to fraud and/or operational abuse of laws, regulations and standards.
· Documents client’s treatment per policy.
· Accurately and efficiently utilizes documentation.
· Reports to work on time and coordinates schedule to achieve maximum efficiency during assigned shift. Maintains regular attendance to ensure clients have access to scheduled programming.
· Maintains safe and clean work area and adheres to company safety standards. Complies with all Infection Control, Universal Precautions, and OSHA standards for the healthcare professional. Attends annual safety and infection control education required by agency.
· Uses safety precautions as appropriate for clients, employees, public and self, e.g. proper positioning of self, good body mechanics, universal precautions, and defensive driving skills.
· Knows the location of all emergency equipment in the facility and is aware of established emergency and safety procedures. Develops and maintains own and client/caregiver knowledge of emergency procedures.
· Demonstrates good judgment when the environment is not physically safe for the staff or the client.
Working Conditions
· Works in office areas as well as throughout the property and its premises.
· Moves intermittently during working hours.
· Is subject to frequent interruptions.
· Is involved with residents, family members, personnel, visitors, government agencies/personnel,etc., under all conditions/circumstances.
· Is subject to hostile and emotionally upset residents, family members, personnel, and visitors.
· Works flexible working hours, including weekends and holidays when necessary, and to keep a pulse on our 24/7 operation.
· Is subject to call-back during emergency conditions (e.g., severe weather, evacuation, post-disaster, etc.).
· Will be involved in community/civic health matters/projects.
· Attends and participates in continuing educational programs.
· Is subject to injury from falls, burns from equipment, odors, etc., throughout the work day, as well as reactions from dust, disinfectants, tobacco smoke, and other air contaminants.
· Is subject to exposure to infectious waste, diseases, conditions, etc., including TB and the AIDS and Hepatitis B viruses.
· May be subject to the handling of and exposure to hazardous chemicals.
· Communicates with other department personnel.
· Maintains a liaison with the residents, their families, support personnel, etc., to assure that the residents’ needs are continually met.
Required Minimum Position Qualifications:
· Degree in Exercise Physiology or related field from an accredited program.
· Personal training experience with ACSM or ACE-certification, including experience working with older adults.
· ACSM group credentialing recommended.
· Must be able to work independently, dependably, and self-motivated.
· Some experience with geriatric individuals recommended.
· Current CPR certification
· A passion for your profession, for clinical excellence, and for applying the latest evidence-based research to your practice.
· A desire for life-long learning and continued professional development.
Specific Requirements
· Must be able to proficiently operate a computer. Must possess basic computer skills in Word, Excel, Outlook, PowerPoint, etc.
· Must possess the ability to make independent decisions when circumstances warrant such action.
· Must possess the ability to deal tactfully with personnel, residents, visitors and the public.
· Must be calm and levelheaded in emergencies.
· Must be well groomed and possess ability to work harmoniously with other personnel.
· Must possess the ability to minimize waste of supplies, misuse of equipment, etc.
· Must possess the ability to seek out new methods and principles and be willing to incorporate them into existing practices.
· Be able to follow written and oral instructions.
· Be knowledgeable in the use of computers, data input/retrieval/output functions, etc.
· Must have patience, tact, cheerful disposition and enthusiasm, as well as must be willing to handle residents, staff, and visitors, based on whatever maturity level at which they are currently functioning.
· Must not pose a direct threat to the health or safety of other individuals in the workplace.
· Must be able to maintain good personnel relations and employee morale.
· Must be able to read and interpret financial records, reports, etc.
