Director of Outpatient Therapy (Physical Therapist) - Charter House | Rochester, MN
Outpatient Therapy at Charter House - Mayo Clinic Retirement Living
Blue Stone Therapy is expanding outpatient therapy services at Charter House – Mayo Clinic Retirement Living in Rochester, MN, and we're looking for a Physical Therapist to serve as our Director of Outpatient Therapy.
This is a hands-on clinical leadership role for a PT who wants to continue treating patients while taking the next step in their career. You'll maintain an active patient caseload while serving as the primary Blue Stone Therapy leader on-site at Charter House, supporting the therapy team and helping shape our growing outpatient program.

What You’ll Do
coordinate the day-to-day operations of the therapy program. You'll serve as a resource for the therapy team and help create a collaborative, patient-focused environment.
Responsibilities include:
- Providing Physical Therapy services in accordance with professional standards and physician orders
- Performing patient evaluations and developing individualized treatment plans
- Supporting day-to-day therapy operations, scheduling, caseloads, and workflow
- Serving as the primary Blue Stone Therapy leader on-site
- Supporting onboarding, mentoring, and development of therapy team members
- Building collaborative relationships with Charter House leadership and interdisciplinary team members
- Monitoring documentation, regulatory, and payer requirements
- Helping identify opportunities to grow and enhance therapy services
- Supporting quality outcomes and a positive patient experience
Requirements:
Qualifications
- Graduate of an accredited Physical Therapy program
- Current Minnesota Physical Therapist license or eligibility for licensure
