Key Responsibilities
Business Development & Growth

Develop and execute a strategic community partnership plan to drive new client admissions and census growth

Identify, pursue, and cultivate referral relationships that convert into consistent business

Meet and exceed monthly and quarterly growth goals
Community & Referral Partnerships
-
Build and maintain strong relationships with referral sources including:
-
Home care, home health, and hospice providers
-
Hospitals, physician offices, and discharge planners
-
Assisted living and senior living communities
-
VA programs, senior centers, and community organizations
Relationship Management
-
Maintain consistent communication and follow-up with referral partners, clients, and families
-
Serve as a trusted resource and liaison within the community
-
Ensure a positive handoff and onboarding experience for new clients
Networking & Brand Representation
-
Represent CareBuilders at Home at networking events, health fairs, and community meetings
-
Increase brand awareness and position the agency as a preferred provider in the market
Reporting & Collaboration
-
Track marketing activity, referrals, and conversions
-
Provide regular updates on performance and pipeline activity
-
Collaborate closely with operations and care teams to ensure excellent service delivery
Ideal Candidate Profile
-
Previous experience in Home Care, Home Health, Hospice, or a closely related healthcare field
-
Established relationships and connections within the local healthcare or senior care market
-
Proven success in healthcare sales, marketing, or business development
-
Strong relationship-building, communication, and presentation skills
-
Self-motivated, goal-driven, and comfortable working independently
-
Organized, persistent, and results-oriented
Qualifications
-
2+ years of experience in healthcare sales, marketing, or business development preferred
-
Knowledge of healthcare referral systems and community-based marketing
-
Valid driver’s license, reliable transportation, and ability to travel locally
-
Experience with CRM or referral tracking systems a plus
