Buckeye Hospice and Palliative Care Services is seeking a compassionate and dependable Licensed Practical Nurse (LPN) to join our team serving patients and families throughout the Sandusky community.

Our LPNs play an important role in providing comfort-focused care and supporting patients and families throughout their hospice experience.

What You'll Do

• Provide nursing care under the direction and supervision of the appropriate clinical team members.

• Assist with patient assessments and implementation of individualized plans of care.

• Monitor patient condition and report changes appropriately.

• Provide medication and treatment support within the scope of licensure and established plan of care.

• Educate and support patients and caregivers.

• Communicate patient needs and changes to the interdisciplinary team.

• Maintain accurate clinical documentation.

• Promote patient comfort, dignity, privacy, and safety.

• Collaborate with nurses and other members of the hospice team.

What We're Looking For

• Current Ohio Licensed Practical Nurse (LPN) license.

• Healthcare, hospice, home health, or long-term care experience preferred.

• Strong communication and clinical skills.

• Compassionate and patient-centered approach.

• Dependable and professional.

• Ability to work independently and collaboratively.

• Valid driver's license and reliable transportation, as applicable.
