✅ About the Facility
Ahava Therapy Services is seeking a Full-Time SLP to join our in-house rehabilitation team at Wexford House, a 173-bed skilled nursing facility offering short- and long-term rehabilitation.

✅Responsibilities

Evaluate and treat residents in accordance with Medicare, Medicaid, and commercial payer guidelines.

Conduct comprehensive assessments to determine swallowing, cognitive-communication, and language deficits.

Develop and implement individualized treatment plans with functional, achievable goals.

Document evaluations, daily treatment, progress notes, and discharge summaries accurately and promptly.

Revise treatment plans based on patient performance and progress.

Educate residents, families, and caregivers on therapeutic techniques and carryover strategies.

Collaborate with nursing staff, interdisciplinary team members, and families to maximize functional abilities.

Participate in care conferences, facility meetings, and quality improvement activities.

Maintain up-to-date knowledge of community resources and assist with discharge planning as needed.

Stay clinically current through continuing education and contribute to in-service education.

Adhere to professional, ethical, regulatory, and facility standards.

Maintain expected productivity levels and demonstrate strong teamwork, communication, and clinical judgment.
✔️ Qualifications

Master’s Degree in Speech-Language Pathology

CFY's allowed to apply
-
Active SLP license liscense in the state of practice

Current BLS/CPR certification
-
Strong observational, verbal, and written communication skills
