Role Description This full-time, on-site Licensed Clinical Social Worker role is based at Nassau University Medical Center in East Meadow, NY. The role involves providing clinical social work services to patients and families, including psychosocial assessments, individual and group counseling, crisis intervention, and discharge planning. The Licensed Clinical Social Worker collaborates with interdisciplinary teams to develop and implement care plans that address medical, behavioral health, and social needs. Daily responsibilities include coordinating community resources, facilitating referrals, advocating for patients, and documenting all clinical interventions in accordance with regulatory and hospital standards. The role also includes participation in case conferences, quality improvement initiatives, and ongoing professional development to support high-quality, culturally sensitive care.\n
Qualifications

  • Demonstrated proficiency in Social Work and Medical Social Work in a healthcare or hospital setting.
  • \n

  • Experience in Case Management, including care coordination, discharge planning, and resource linkage.
  • \n

  • Knowledge and experience in Mental Health, providing assessments, counseling, and crisis intervention.
  • \n

  • Background in Behavioral Health, including working with diverse behavioral and psychosocial conditions.
  • \n

  • Active Licensed Clinical Social Worker (LCSW) credential in New York State, or eligibility according to state requirements.
  • \n

  • Master’s degree in Social Work (MSW) from an accredited program.
  • \n

  • Strong communication, documentation, and interpersonal skills; ability to work effectively within interdisciplinary teams.
  • \n

  • Commitment to cultural competence, trauma-informed care, ethical practice, and patient advocacy.
  • \n

  • Experience in a hospital or acute care environment and familiarity with public health systems is preferred.
