ESSENTIAL DUTIES AND RESPONSIBILITIES

Employees are expected to perform essential duties to performance standards.   • Demonstrates safe driving practices and is a focused, confident, and observant driver.
  • Maintains a safe driving record and follows all federal, state, and local driving laws, rules, and regulations.
  • Follows verbal directions and ability to utilize navigational maps, GPS systems, and vehicle manuals. Reviews and plans routes in advance.
  • Ensures that the specialized systems on the unit are maintained through scheduled maintenance.
  • Completes pre-trip inspection for mobile unit’s reliability, and ensures that the unit is complies with all regulations.
  • Performs basic trouble shooting and takes to mechanic when needed.
  • Reports expected NOAA weather changes, watches, or warnings to direct supervisor prior to mobile unit use and follow directives accordingly.
  • Assists to secure the mobile unit and seek indoor shelter when faced with an unexpected severe weather-related emergency.
  • Responds to an unexpected community disaster as directed by supervisor.
  • Ensures safety during events, including safe entry, parking, and exit of the mobile unit.
  • Monitors parking and storage environment of mobile health unit.
  • Assists patients/visitors with safe entry and exit on the mobile unit when needed.
  • Implements and evaluates operational and administrative processes.
  • Participates in aspects related to quality compliance, improvement, and risk management.
  • Engages in corporate compliance requirements.
  • Coordinates with clinicians to ensure adequate supplies are prepared and stored on unit properly prior to departure from site or treatment location.
  • Maintains accurate written and electronic records of mobile unit’s mileage and clinic locations, pre-and post-trip forms, daily vehicle usage log, AED safety log, fire extinguisher logs, and maintenance logs.
  • Assists clients to navigate and access medical care, community services/resources and to develop healthy behaviors.
  • Acts as a caring neighbor to help clients address the social, medical, and community problems that lead to poor health.
  • Collaborates with medical providers to help clients deal with the ‘real life’ issues that keep them from staying healthy, help with tasks of getting medical care, and work on health goals by doing things like planning healthy meals and finding time to exercise.
  • Provides social support, advocates for individuals and community health needs.
  • Conducts needs assessment by gathering information from the client, family and/or medical provider. Assists patients to set healthy goals by working cooperatively with medical providers, care coordinators, and/or health plans to help ensure clients have comprehensive and coordinated care. Documents each client encounter in detail according to protocols and guidelines.
  • Establishes trusting relationships with clients and their families while providing general support and encouragement including motivating clients to meet their health goals by utilizing harm reduction techniques.
  • Corrects and updates all necessary data to assure timely, accurate bill submission. – Verification
  • Provides consistent communication to program supervisor to evaluate client/family status, ensuring that provided information and reports clearly describe progress and meets regularly with Supervisor to review priorities, project status, and discuss interventions and client needs.
  • Exhibits excellent working relations with clients, visitors, and other care team members.
  • Maintains confidentiality of HIPAA and other confidential and sensitive information.
  • Maintains the highest level of integrity and professionalism in all aspects of job performance.
  • Attends and participates in required trainings on topics relevant to the job and to behavioral healthcare.
  • Performs job responsibilities and maintains all records in accordance with BHP’s policy, procedure and protocol, and as appropriate to the job, all regulatory authorities’ accreditation/licensure/

certification standards, and all applicable laws and regulations.

  • Works assigned schedule, exhibits regular and punctual attendance and works outside of normal schedule as required.

EQUIPMENT OPERATED:

Computer, cell phone, copier, fax, and other general office equipment; larger vehicles, i.e. delivery trucks, multi-passenger vehicles, bus, etc.

CONTACTS WITH OTHERS:

Clients, staff, visitors, community professionals, volunteers and vendors.

CONFIDENTIAL INFORMATION AND DATA:

Client information and other confidential and sensitive material.

WORKING CONDITIONS:

Good working conditions when working in the mobile unit, due to limited space. When working in the mobile unit, the employee is exposed to conditions within that facility. May be exposed to hostile or dangerous clients and other hostile or dangerous job contacts. May be exposed to contagious diseases. Possible exposure to inclement weather and traffic conditions when traveling to and from worksites. Ability to adjust to health related or inclement weather situations, or as required by management.

USUAL PHYSICAL DEMANDS

The following demands are typically used and exhibited by employees when performing this job’s essential duties and responsibilities, They are not, and should not be construed to be qualification standards. The employee frequently converses verbally with others in person. The employee regularly lifts up to 50 pounds. The employee regularly exhibits digital dexterity and eye-hand coordination when performing essential job duties. The employee will be sitting, standing, and walking approximately 80% of the time. Employee will be bending and reaching approximately 20% of the time. Normal accessibility and mobility throughout the region required. The employee frequently sits in a vehicle for periods of time while riding to and from multiple sites.

REQUIRED KNOWLEDGE, SKILLS AND ABILITIES

Knowledge of: Will need a basic understanding of clinical/medical terminology; accreditation and certification standards, health and safety codes/requirements, repair/maintenance techniques and safety standards. Knowledge of local community resources (housing, food, support groups, financial assistance programs, utility assistance programs, etc.).

Ability to: collaborate with clinical professionals and medical providers or other members for the care team; establish rapport with staff, clients and vendors; work with diverse populations; respond to staff questions; coordinate multiple activities and tasks; exhibit a pleasant public manner; be self-directed and organize the work day in a manner that allows one to prioritize tasks and accomplish organizational goals; understand and follow instructions, develop and maintain effective working relationships with co-workers and other job contacts; demonstrate respect and sensitivity for cultural and personal differences; contribute toward building a positive and productive team working environment; maintain confidentiality of confidential and sensitive information; consistently exhibit job competency, ethical conduct, integrity and trustworthiness; embrace and effectively adapt to organizational change; exhibit behavior and conduct consistent with BHP values and policies.

QUALIFICATIONS

Applicable laws or regulations require that employees assigned to some jobs attain a specific level of education, experience, or training, and license or certification. For other jobs, a combination of various levels and types of education, experience, training, and license or certification may qualify a person to perform essential duties of this position. Qualifications specified for this position are:

Education, Experience and Training

High School diploma (or equivalent) and previous driving experience (training may be provided). Previous professional experience driving preferred. Experience in outreach, customer service, social service, or a related field.

Licenses or Certifications

Qualified Mental Health Specialist (QMHS). State of Ohio Driver’s License with clean driving record, preferred CDL. BLS/CPR certification required.

Additional Qualifications

Upon conditional hire, and as a condition of employment, each employee is required to submit to and pass a drug test, criminal background check, and may be required to submit to driving record check, personal vehicle inspection, and be insurable under BHP’s driving insurance provider guidelines depending on job assigned job duties.

Ability to document personal identity and employment eligibility within three days of original appointment as a condition of employment in compliance with Immigration Reform and Control Act (I-9) requirements.
